ADL Assessments


Activities of Daily living Assessments (ADL Assessment)

Where an individual’s capacity for employment has been significantly impacted by their injury/illness there is likely to have been a corresponding impact upon their capacity for domestic, recreational and community-based activities. An ADL or household assessment is conducted to identify the specific problems and prescribe alternative equipment or recommend appropriate levels of household services.

 

ADL Assessment and services can include:  

Household help & Gardening Review and Strategies

  • training in body mechanics

  • service recommendations

  • gardening equipment

  • adapted cleaning equipment

equipment needs

  • mobility aids

  • bathroom seating

  • recliner chairs

  • bedding review

Home modification needs

  • transport assessments

  • car modifications

  • taxi request

The recommendations are discussed with the treating doctor and the assessment is submitted to the insurer for their consideration. 

There are no out-of-pocket expenses as this assessment is in accord with their existing policies of the TAC and Worksafe.

Therefore, if you are aware of your client’s experiencing difficulties at home due to their compensatable injuries refer them to Surfcoast Occupational Therapy.

 

Self Referrals

Worksafe Claimants

Currently Worksafe claimants require a referral from their treating Doctor to obtain Occupational therapy services.  

Therefore, make an appointment with your Doctor to discuss your problems your experiencing at home and direct them to our on-line referral form. We will then contact your claims officer and call you back to make the appointment.

 TAC Claimants

TAC claimants do not require a referral from a Doctor and therefore can fill out the Self-referral form below.
